It was a very good learning experience for me at AWWA Senior Community Home on 29 May 2023. AWWA Senior Community Home is a care facility where seniors live together and receive nursing care all through the day and night. Depending on their health statuses, seniors are empowered to maintain various degrees of independence. Some still do their own cooking and run their own errands, while some take part in social activities within the facility.
Thanks to the close partnership between Pancare and AWWA, iRx had the opportunity to partake in the care of these seniors. In this session, we collaboratively review the seniors’ health problems, simplify the care plan and medication regimens, monitor therapy and deliver their medications to them. From a pharmacist’s point of view, there are definitely many opportunities for community clinical pharmacists. There are so many unnecessary drugs to deprescribe and complicated medication regime to simplify. We can also streamline the patient’s care and save them on multiple visits to different public health institutions.
This collaboration sets an excellent platform for a cohesive multidisciplinary health team for managing patient medical and social prescription in seamless manner. Looking forward to our next session on 7 Jun 2023!
