Super honoured to be speaking at this upcoming event on Social Prescribing!
What is social prescribing? According to NHS, it is an approach that connects people to activities, groups, and services in their community to meet the practical, social and emotional needs that affect their health and wellbeing.
The idea of social prescribing is definitely not new, even though social prescribing does sound like a big term. To put it in simple words, it's about linking our clients up with the relevant services, groups and activities to improve their overall happiness. Clinicians in the primary care setting, i.e. all medical doctors, nurses, pharmacists and allied health professionals can do this on a day-to-day basis!
At POPCollect 2023, I will be speaking on behalf of PanCare Medical Clinic and iRx Clinical Pharmacy on specific examples of how social prescribing can be done in the primary care setting and how partnership with community care partners can further enhance the medical and non-medical care for the residents!
